Suggest you go to the menu on the printer and go the reports menu. Try to print a config page or a demo page and see if it does the same thing. If it does not, then the issue is the driver and you probably need to remove and reinstall the printer on the computer. If it prints the same thing, I would think the formatter has an issue and needs to be replaced.
